Athletes need to call to schedule their first appointment one week 
before they are ready to start the program.

GROUP RATES: In order to receive a group rate, all pre-registration
forms must be returned together with all payments.

_____2 or 3 Athletes $435.00 each

_____4+ Athletes $420.00 each
